ROBOTIC ASSISTED RESECTION OF INTRATHORACIC SUPERIOR MEDIASTINAL PARATHYROID ADENOMA

This video demonstrates robotic assisted resection of ectopic parathyroid adenoma situated in superior mediastinum. Patient was a 39 year gentleman presented with renal stone and on work up detected with hyperparathyroidism (S.PTH 1503). Parathyroid scan suggested superior mediastinal adenoma in thorax. Contrast enhanced CT scan thorax confirmed the 4 cm lesion in superior mediastinum. Da Vinci Xi system is used to perform this procedure. Patient position is supine position. 3 robotic ports and 1 assistant port were used for the procedure. At the end of resection specimen was sent for frozen section and confirmation of parathyroid tissue done. After 20 minutes, S. PTH level evaluated which came to 239. (preoperative level was 1503) Specimen extraction was done by extending assistant port. Post op. recovery was uneventful. Hospital stay was 72 hours.
