Infection below the apex of the tooth occurs due to various reasons. Tooth caries , periodontal infection, Trauma from occlusion plays a major role among them. Maxillary anterior tooth infection mainly occurs due to caries spreading from enamel to dentin, involving the pulp chamber and finally infiltrating the root area. Mandibular anterior tooth infection mainly occurs due to trauma from occlusion.And this results in periapical pathology which is seen after 6 months to 1 year. The treatment protocol follows a standard rule. Root canal treatment of the involved teeth , followed by apical curettage or enucleation of the lesion with or without apicectomy and retrograde obturation. The defect can be filled with bone grafts or PRF.
