This patient has been associated with the San Diego Headache and Facial Pain Center for the past 7 years. His condition developed over some time where he received many dental consults. Eventually his internist referred the patient to Dr. Eli. Trigeminal Neuralgia is treated in the practice with a combination of injections and other medications. The patient was pain free and managing his pain on his own for over 5 years. Recently the pain has returned so a new consultation was sought. Dr Bradley Eli: Welcome back we are seeing you again...we first saw you how many years ago? Patient: Probably 7 years. Dr Bradley Eli: & years ago what was your problem? Patient: Pain in my cheek. Dr Bradley Eli: How would that pain come on? Patient: Usually from tongue movement or a light touch on my cheek, ir my chin and lips. A light touch would trigger it. Dr Bradley Eli: When it would trigger it can you describe for us what the pain was like? Patient: More of an intense burning pain. Dr Bradley Eli: How long did it last when it came on? Patient: The pain would be momentary maybe a few seconds to maybe ten seconds or so. Dr Bradley Eli: So you went to some dentists because your tongue touching your teeth triggered it? What did the dentists say? Patient: I went to several different dentists because I was sure it was my molar, and it needed to be pulled, or something. It turned out there was nothing wrong with my teeth so they referred me to my internist and he basically diagnosed it as Trigeminal Neuralgia. He sent me to you. Dr Bradley Eli: OK and where are you at now? Patient: I had seen you really regular many years ago, for some reason it was just fine and I figured I didn't need treatment anymore. So I didn't I went for about 5 years without seeing you. Now I am back, the pain was gone when I saw you I got some shots. You gave me some medicine and the pain was gone. Dr Bradley Eli: And so were you... Patient: And I was very happy. I was still taking the medicine like a good boy. Now the pain is back. Dr Bradley Eli: Alright so we are going to give you some shots again and we are going to see if we can capture your pain. You have Trigeminal Neuralgia that's what we are calling what you have.
