Stapedectomy Vs Stapedotomy || Which is better || ENT Lecture Otosclerosis - • Otosclerosis || Otospongiosis || ENT Lecture Stapedectomy and stapedotomy are surgical procedures performed to treat a condition known as otosclerosis, which affects the small bone in the middle ear called the stapes bone. Otosclerosis can cause hearing loss by preventing the stapes bone from vibrating properly. Both procedures aim to restore or improve hearing by addressing this issue, but they differ in their surgical techniques. . Stapedectomy: • Stapedectomy is the older of the two procedures and has been used for many decades. It involves the complete removal of the stapes bone, which is then replaced with a prosthesis made of either plastic or stainless steel. The prosthesis is designed to transmit sound vibrations from the middle ear to the inner ear, bypassing the malfunctioning stapes bone. • The procedure is typically performed under local or general anesthesia, and it requires making an incision in the ear canal to access the middle ear. After the stapes bone is removed and replaced with the prosthesis, the incision is closed, and the patient is monitored during recovery. • Stapedectomy is considered highly effective in improving hearing and is often the preferred choice for patients with otosclerosis. Stapedotomy: • Stapedotomy isa more modern and less invasive alternative to stapedectomy. Instead of completely removing the stapes bone, only a small hole (fenestration) is made in the footplate of the stapes bone. This hole is then enlarged, anda prosthesis is inserted into it. • The prosthesis in a stapedotomy is typically a piston-like device, often made of titanium, which is attached to the incus (another small ear bone) or directly to the oval window, a membrane leading to the inner ear. • Stapedotomy is performed through the ear canal, similar to stapedectomy, and is also done under local or general anesthesia. • The advantage of stapedotomy over stapedectomy is that it tends to preserve more of the natural anatomy of the ear. This can result in fewer complications and a quicker recovery for some patients. Both stapedectomy and stapedotomy can significantly improve hearing in individuals with otosclerosis. The choice between the two procedures depends on the specific characteristics of the patient's ear and the preferences of the surgeon. Post-surgery, patients often experience improved hearing and a reduction in symptoms like tinnitus (ringing in the ears) associated with otosclerosis. However, as with any surgery, there are potential risks and complications, so it's essential to discuss these options thoroughly with an ear specialist or otologist to determine the most suitable approach for each individual case. #earinfection #entlive #mbbslectures #mbbsfighters
