This is a video showing a vitrectomy and laser being performed for a patient with bleeding in the eye and a traction retinal detachment, from diabetes. I have narrated this unedited procedure. The first phase involves making three small openings in the eye. The first opening is to allow for the placement of an infusion line, while the other two allow for the the insertion of instruments for each of my hands. Underneath the operating table, away from view, are several foot pedals. One set is for control of the microscope, allowing me to move it in different directions, and the other is for control of the machine that tells the instruments in the eye what to do. Sometimes, there is a third pedal, that controls the lens that we use during surgery. After the openings are made, and the cannulas are placed, the central vitreous is removed, and in cases where a patient has a traction retinal detachment, like this one, great care is taken to relieve the traction. Next, I inject some steroid in the eye, to help identify the remaining vitreous, and carefully create a posterior vitreous detachment. I then remove as much of the remaining vitreous as possible. I then apply laser to the retina; the laser helps reduce the risk of future bleeding. I then perform a scleral depressed exam, to inspect the retina for any tears that may have been created during surgery. Lastly, I replace the fluid in the eye with air, remove the cannulas, administer some antibiotics, and conclude the case.
