In this powerful episode of "Stories from the Field," we connect with Dr. Jack Wren, a neonatal intensivist particularly interested in understanding neonatal physiology. Dr. Wren discusses his groundbreaking published study on using early echocardiography (heart ultrasound) for critically ill newborns with congenital diaphragmatic hernia (CDH). These echos are often delayed, forcing teams to manage incredibly ill babies blindly. Dr. Wren's research demonstrated that by having highly-trained neonatologists perform a rapid, 15-minute ultrasound much earlier, at an average of three to four hours of life, it could be done safely without causing clinical instability. Dr. Wren used Sickbay data to provide the essential evidence for his study. By leveraging continuous, near real-time vital sign monitoring trends, he could prove that the early echo approach was safe, showing a "straight line" in the babies' vital signs during and after the procedure. This work not only changed care practice at his hospital, but faculty are also now using Sickbay’s continuous monitoring to supervise procedures remotely from their offices, enabling better oversight when they cannot be physically at the bedside. Dr. Wren and his team are now focused on their next goal: proving that this safe, early intervention will ultimately improve outcomes for the three out of ten CDH babies who currently do not survive to discharge.
