Various guests from all over India and the world were invited to share their expertise and experience on different head and neck cancers. Speaker: Dr. Saumya Matthews Topic: Inoperative Challenges in Microvascular Free Flap Surgeries In this video, Dr. Saumya talks about: a. Pre-reconstruction intraoperative checks and team coordination Before addressing the defect, ensure the resection team has finalized margins and completed neck closure. Verify drains and tubes are secure, and involve the head and neck team for maxilla-mandibular fixation if needed. Intraoperative defect assessment Accurate assessment must consider three dimensions, not just two, to avoid postoperative issues. Identify all components needing replacement and understand their functional and reconstructive goals. b. Donor site considerations and avoiding morbidity during flap harvest Recognize that donor site appearance and healing vary greatly among patients, especially in the Indian context. Aim for minimal donor site morbidity, preserving function and ensuring primary closure whenever possible. c. Tailoring reconstruction to specific sites Reconstruction strategies must be adapted to the unique requirements of each head and neck subsite, like the buccal mucosa or lip. Consider functional outcomes, cosmesis, and potential complications associated with different flap choices for each area. d. Salvage strategies for flap failure and management of vascular issues Flap failure necessitates a decision on replacement with another pedicled or free flap, addressing issues like bleeding or infection. For vessel compromise, techniques like the calic flip procedure and utilizing alternative vessels are crucial. e. Essential intraoperative measures Maintain adequate intraoperative relaxation and closely monitor blood pressure to ensure flap viability. Have necessary vaso-dilatory drugs, anticoagulants, and properly maintained microvascular instrumentation readily available, and utilize adequate magnification.
