This video is about V Groove Technique with Hydro-dissection for Routine Phacoemulsification surgey The V groove (also known as the "victory") technique was described by Dr Kelman in 1994 in the days before the continuous capsulorhexis. It involves sculpting the nucleus in the shape of a V (2 grooves joined in the subincisional area and formed a V shape) This is followed by cracking along both arms of the V and then removing the central piece first. The other two pieces are removed easily after that with or without chopping. This offers an advantage of not stretching the capsule while removing the pieces V Groove technique without hydro-dissection is a quick and safe technique in cases of: 1. radial anterior capsule tear 2. injury from vitrectomy cutter 3. posterior polar cataract “when you want to avoid or suspect a posterior capsule tear” As suggested by Tom Oetting, Brian Samuels and Jeff Yee I do V Groove without hydro-dissection only in the above mentioned cases. In my experience V Groove technique following hydro-dissection is an easy, quick and safe technique for routine phaco and IOL implant surgery. I use it routinely and successfully in all my cataract cases with different grades of cataract density and in cases with small pupils and floppy irides. as you can see in the videos
