The pancreas receives a rich arterial blood supply through multiple branches derived from both the celiac trunk and the superior mesenteric artery, ensuring adequate perfusion for its exocrine and endocrine functions. The vascular supply is complex and involves numerous anastomotic connections that contribute to the organ’s resilience against ischemic injury. The superior pancreaticoduodenal artery, a branch of the gastroduodenal artery, plays a crucial role in supplying the upper portion of the pancreas, including the head and uncinate process. This artery further divides into anterior and posterior branches, which form extensive anastomoses with the inferior pancreaticoduodenal artery. The inferior pancreaticoduodenal artery arises from the superior mesenteric artery and supplies the lower portion of the pancreatic head and the duodenum. These pancreaticoduodenal arcades form an essential collateral circulation between the celiac trunk and the superior mesenteric artery. The body and tail of the pancreas receive their blood supply primarily from the splenic artery, a major branch of the celiac trunk. The splenic artery gives off multiple pancreatic branches, including the dorsal pancreatic artery, great pancreatic artery (also known as the arteria pancreatica magna), and the caudal pancreatic artery. These branches form a network of anastomoses that ensure the extensive vascularization of the pancreas, particularly along its posterior and inferior aspects. This detailed knowledge of the pancreatic arterial supply is critical for surgical procedures involving the pancreas, including pancreaticoduodenectomy, distal pancreatectomy, and interventions for pancreatic trauma or tumors. Understanding the arterial variations is essential for preserving pancreatic function and minimizing postoperative complications. In this video, Dr. Gunjan Desai, DNB Teaching Faculty for gastrointestinal, liver, and pancreatic surgery at Lilavati Hospital, Mumbai, provides an in-depth explanation of the arterial supply of the pancreas, highlighting key anatomical landmarks, surgical relevance, and clinical implications.
