SystmOne Social Care enables services to share vital information about a client with other clinicians involved in their care. The module is designed to save time and enhance the delivery of care using functionality that enables the easy recording of assessments and comprehensive safeguarding provision. In response to the increasing emphasis on integrated and client-centred care, SystmOne Social Care provides the missing piece of the puzzle. Workflow - Configurable workflow throughout the system to map to new or existing processes. Full audit trail of care provided. Highlight import task or new actions due using colour co-ordinated traffic light system. Shared Care - Share information securely with the SystmOne estate with the client's consent. Avoid duplicating information, saving time. Reduce wasted visits. All data captured once and used many times avoiding repetition. Assessments - Record assessments for the clients and carers, including questionnaires and outcomes. Report on assessment outcomes, ensuring targets and statuary returns are completed. Manage review dates to monitor clients progress. Filter and sort caseloads by review dates to make sure they carried out. Costing - Other services can be added to the clients records based on recording needs. Individual services can be configured in the system. Can support the management of budget and payment via SystmOne's integration with ContrOCC. Contact Management - Contacts can be recorded in the system with out the need for a referral. Adding a referral will allow other services to be added like assessments and caseload management. This is a quick and easy process to add a new contact. Other services can refer directly into the Social Care module allowing seamless flow of information between services. Safeguarding - Contacts can be make via the safeguarding website if users are concerned about the safety of a client. Contacts created directly into social care module can be assessed by a member of staff. Ability for users to calculate and record an individuals risk with SystmOne. Airmid - Our client-facing app which can support enablement of self-management. Clients can book appointment, view their own record and completed assessment with inbuilt decisions support. Results of assessments can be viewed within the record and reporting on. Proxy access can also be given to family members or carers if required.
