Educational video describing the condition of posterior sternoclavicular joint dislocation. The sternoclavicular joint is supported by strong ligaments. Dislocations of the SC joint can occur due to injury to the ligaments. Posterior dislocation is dangerous. It can affect the trachea, esophagus, veins and arteries. If posterior dislocation of the SC joint is suspected, CT scans are best for assessment of associated injuries and establishing the diagnosis. This CT scan showed posterior dislocation of the clavicle in a patient who sustained a shoulder injury 3 days prior to obtaining the CT scan. The patient was evaluated because of swelling of the left upper extremity and Doppler showed DVT of the left upper extremity and CT with contrast was ordered. The CT scan unexpectedly showed the clavicle to be posteriorly dislocated, compressing the brachiocephalic vein and causing DVT. Treatment •Urgent open reduction was done with backup from a cardiac surgeon •The patient did very well. •The joint is usually stable once reduced. This case is probably a posterior fracture dislocation of the medial clavicle since the medial epiphysis is the last one to close. Posterior dislocation can be a difficult diagnosis. Could be associated with dyspnea, dysphagia or tachypnea and compression of the great vessels. In our patient, it was associated with a DVT of the upper extremity. On the other hand, anterior dislocation is common, obvious, benign and does not affect the function.
