I presented this case a a Surgical conference in 2023. I thank Mr Aziz Momin for the invitation to speak again. It is a rare case, and one with a good result at the end. I take you through to options and how we did this tricky procedure. A Gerbode Defect links the left side of the heart (pumping chamber) to the right atrium (collecting chamber) . This a high pressure jet of blood is going "backwards" and can leaft to heart failure. This patient had a tough initial surgical procedure to replace the aortic valve. This created a Gerbode defect, and he became highly sympmtomatic. This is a very rare complication of cardiac surgery. It can also happen after infection or even trauma. He was not born with it, although it can be a congenital abnormality. What options are there? 1. Medical therapy- we could give diuretics (water pills), but that would not cure the problem. And the right side of the heart would come under strain, and then fail. 2. Redo Surgery. This would be complicated, and he struggled during the 1st surgery. Thus high risk. 3. Plug the leak. Not every center has the skill base to do this, but I have acquired the skills to do this over the years. Our patient has done well with the plug, and feels much better.
