Often found near the head of the pancreas (important in transverse epigastric view). Use color Doppler to distinguish the CBD (nonvascular) from vessels. Measurement Steps: Identify the CBD in Longitudinal View Ensure you’re seeing the CBD in long axis. CBD should appear as an anechoic tubular structure with echogenic walls. Use Color Doppler: Turn on color Doppler to differentiate vascular structures (portal vein and hepatic artery) from the CBD (which will not show color flow). Freeze the Image at a Clear CBD View: Look for the CBD at the porta hepatis, or more inferiorly near the pancreatic head. Freeze when you get a clear, well-defined image of the CBD lumen. Place Calipers: Measure the inner diameter of the CBD. Place calipers from inner wall to inner wall (not outer-to-outer). Normal Range Less than 6 mm (general population) Up to 7–8 mm in post-cholecystectomy patients Add 1 mm per decade after age 60 as a rough rule of thumb
