Polar cataracts are one of the most difficult to do due to the risk of having defect in the posterior capsule (PC). This is the only video I have with true PC defects as the others did not have it. I tried hydrodelineation, first sculpt then hydrodelineation, without hydrodissection however the best technique I found is gentle peripheral hydrodissection followed by gentle visco-dissection. This video of a patients in his 30s, it shows a running out capsulorhexis and its retrieval, removal of a polar cataract and leaving the central SLM to the end, and using Simcoe cannula. Since then, I moved to bimanual I/A and gentle peripheral hydrodissection and visco-dissection. Vision blue used in case needed vitrectomy
