The double rhexis technique is a method used primarily in cataract surgery for challenging cases like intumescent white cataracts, where there is increased intralenticular pressure due to liquefied lens cortex. This technique involves first creating a small initial capsulorhexis (a small opening in the anterior capsule of the lens). This small opening allows safe decompression of the capsular bag by releasing the liquefied contents and reducing pressure, which helps prevent the capsular tear from running outwards—a complication referred to as the “Argentinian Flag sign.” After decompression, the initial small capsulorhexis is carefully enlarged by making a second, larger capsulorhexis to the desired size (usually around 5 to 5.5 mm). This controlled, two-step approach reduces the risk of capsular tear extension and allows safer completion of the cataract surgery with nucleus removal and intraocular lens implantation. The key advantage of the double rhexis technique is better control and prevention of capsular complications in high-pressure cataracts, making it a favored approach for white intumescent cataracts.
