Let us understand what is azoospermia, its types, and what are the causes of obstructive azoospermia. What is azoospermia? Azoospermia means zero sperm count or nil sperm count, which needs to be proven in two successive reports, which are done approximately 4-6 months apart. Types of Azoospermia -Non-obstructive azoospermia -Obstructive azoospermia Male reproductive system Before understanding what causes of this obstruction lets understand the male reproductive system 1. Testes, commonly known as testicles, is the part of the male reproductive organ where sperms are produced. 2. Later these sperms travel via a duct or a pipeline called the vas deference and are temporarily stored in seminal vesicles. 3. In the seminal vesicles, certain nutrition and fluids are added to these sperms. 4. At the time of intercourse, the sperms are ejaculated through ejaculatory ducts from the penis and it gets out as the seminal fluid. In the case of obstructive azoospermia, there is a blockage in the epididymis or vas deference or elsewhere along the reproductive tract. What is obstructive Azoospermia? The testes are normal, which implies that the production is occurring normally, but the sperm produced somehow can not come out due to the blockage or absence of the duct. This is obstructive azoospermia. When a person has obstructive azoospermia because the testes are normal. The hormonal levels which regulate sperm production are going to be normal. Causes of Obstructive Azoospermia? 1. Obstruction or blockage could be because of genital infections that block the vas deference. 2. Blockage or choke-up of ejaculatory ducts. 3. In some rare cases, there can congenital absence of this duct which is known as Congenital Bilateral Absence Of Vas Deference. Treatment for obstructive Azoospermia 1. Microsurgical techniques available for very specific indications but the blockage is because of infectious causes than is virtually impossible to treat these though surgical techniques. 2. TEFNA (Testicular Fine Needle Aspiration) also called an epididymal aspiration of sperm, is a procedure performed for men who are having sperm retrieved for IMSI/ICSI who have obstructive azoospermia.
