This video showcases a challenging laparoscopic total hysterectomy in a patient with a history of pelvic inflammatory disease (PID), dense pelvic adhesions, and two previous lower segment cesarean sections (LSCS). Key highlights of the case include: • A right tubo-ovarian mass (TO mass) densely adhered to the right pelvic wall. • Right-sided retroperitoneal dissection was performed to identify and safeguard the ureter, followed by division of the infundibulopelvic ligament. • Due to the TO mass being stuck to the right pelvic sidewall, we opted to divide the cornual ligament complex first. • On the left side, the sigmoid colon was adherent to the lateral pelvic wall. We carefully dissected retroperitoneally to identify the left ureter. • The harmonic scalpel was employed precisely in areas of dense adhesion near the bowel and bladder, minimizing thermal spread and risk of injury. This case emphasizes the importance of meticulous dissection, ureteric identification, and strategic energy device use in complex pelvic surgeries.
