This is a benign sweat duct tumor, in this case, a poroma (por - duct, oma - tumor). Acrospiroma = Benign Sweat Duct Tumor: Benign sweat duct tumors are derived from some portion of the ducts that carry sweat to the skin surface. Most of the tumors in this family are derived from the acrosyringium (intraepidermal portion). Although not perfect, the easiest way to think about this family of lesions is by the location of the tumor within the skin: 1. Limited to the EPIDERMIS - Hidroacanthoma Simplex (like bowens) 2. Within the UPPER DERMIS in nested nodules - dermal duct tumor (like micronodular basal cell) 3. Larger with MULTIPLE CONNECTIONS TO EPIDERMIS - poroma (like endophytic seb ker) 4. EVEN Larger tumor in dermis often with cystic spaces, clear cells, may be hypervascular - hidradenoma, solid and cystic and clear cell types. The cells that make up these tumors look quite similar (hidradenomas may look different if they are an apocrine, clear cell, etc..): Micro Keys: 1. Bland polygonal to round cells with abundant pink cytoplasm and vesicular (can see through the nucleus). 2. Minimal pleomorphism (most cells look similar to each other) 3. Cuticle (pink keratin) lined ducts usually present 4. Few if any mitotic figures 5.
