Minimally Invasive Sinus Augmentation with Sinus Membrane Removal Sinus augmentation surgery has made significant advances over recent years. The biggest advances have been a progression toward minimally invasive surgery. Traditional methodology has been shown to result in 50% of grafts becoming infected that either require prolonged antibiotics or the need to remove the graft material in 5% of sinus augmentations. Not only is minimally invasive surgery less traumatic on the patient, but reducing the size of the osteotomy and delivering the graft material via sterile delivery systems eliminates post operative infection of the sinus membrane because it is removed for the procedure. The sinus membrane then grows back naturally over the graft material. Bone Graft Science The dental profession defines bone quality on the hardness of the bone. The harder the bone, the higher the bone quality. However, cadaver bone grafts produce sclerotic bone that is hard and high in mineral content. Virtually every dentist is taught to use cadaver bone graft material even though good science tells us it is not supporting the implant, which leads to progressive marginal bone loss over time or implant failure. Hardness is not an acceptable method of defining bone quality--the biology of the bone should be the priority. The goal of any implant method is to produce 100% bone-to-implant integration with healthy normal bone that will last the life of the patient. But just because your radiograph appears to show mineralization does not guarantee that the tissue is integrated to your implant or is even healthy. Cadaver bone grafts look good on a CT scan because they blend in with your existing bone, but our professors and lecturers do not understand what is happening biologically. Cadaver bone graft granules, according to the scientific literature, are never fully resorbed and do not produce integration. "Evidence-based dentistry" is a common talking point, except when it runs into dogma. Dentistry has studied bone grafting procedures extensively for 40 years, but the profession has not studied bone biology or bone graft biology. Because of this, the dental profession knows very little about the bone we are placing our implants in and even less about the bone that we create when a bone graft is placed. For the last 20 years, we have exclusively studied bone biology and bone graft biology and we do understand the bone that we place our implants in as well as the type of bone that is created by not only our science-based products, but also how cadaver bone grafts mineralize and fail. Our industry leaders have been promoting cadaver bone grafts for the last 30 years and are unable to accept that cadaver bone grafts are inferior to modern science-based bone grafts.
