We present one of the few possible phacoemulsification techniques for the posterior polar cataract, in a high myopic eye which underwent radial keratotomies 20 years ago. We highlight the importance of avoiding the hydrodissection, the nucleus and epinucleus rotation and the anterior chamber flattening. Sufficient time and patient must be allowed to the epinucleus and cortex aspiration. OVD must be injected in the AC several times, prior to every attempt of phacotip or irrigation canula withdrawal. The surgery is performed with Centurion machine and we implant an Arysof SN60WF IOL, with the automated injector system Autosert. At the end, UNDER the in-the-bag implanted IOL, we perform a small posterior capsulorhexis, in order to obtain a complete clear central visual axis. No financial interest. Cataracta polara este intotdeauna o provocare pentru chirurg. Este necesara o tehnica chirurgicala ireprosabila, pentru a evita riscurile foarte mari de spargere a capsulei posterioare cristaliniene. In acest caz, pacienta are si keratotomii radiare efectuate in urma cu 20 de ani, pentru miopie forte.La sfarsit se implanteaza un cristalin artificial foldabil, cu ajutorul sistemului automat Ultrasert, ceea ce confera implantarii un control maxim.
