Immediate Rehabilitation in Atrophic Maxillae Course | CIMIV Institute

On 19th and 20th April, we taught the 2nd edition of the course: Immediate Rehabilitation in Atrophic Maxillae at the CIMIV Institute and we thought it was a good opportunity to explain the advantages of immediate loading on implants for both professionals and patients. As you know, the aim of the course is to train our colleagues in the rehabilitation of the atrophic maxilla by placing tilted implants or implants anchored in anatomical butresses for their immediate loading. This type of treatment is an alternative to other more invasive of bone augmentation procedures. One of the main advantages of these techniques is to reduce the time of treatment. Using bone augmentation procedures, the patient often has to undergo several surgical interventions, which delays the duration of treatment and depending on the cases can reach a year or more. In this type of treatment, the placement of an immediate prosthesis on the implant is usually contraindicated, so that, in addition to lengthening in time, the quality of life of the patient throughout the process is worse. During the months that the treatment is prolonged while awaiting the integration of the bone graft, the patient remains edentulous. This, obviously, is a problem for both aesthetic and functional reasons. Temporary removable prostheses are not always a solution since many patients do not adapt to them. In oral rehabilitation treatments in which an immediate loading is made, the patient recovers the masticatory, aesthetic and phonatory functions from the beginning of the treatment. It is important to clarify that not all patients are suitable for this type of treatment and will depend on the primary stability of the implants, the quality and quantity of the maxillary or mandibular bone and individual patient considerations. That is, it will depend of the tobacco habit, the state of periodontal health, if the patient suffers some type of systemic pathology, such as cardiovascular diseases, anticoagulant treatment, etc. However, for those patients whom an immediate loading of their implants is possible, this treatment allows them to recover the aesthetics of their smile and the functionality of their masticatory system in 24 hours. Although the procedure obviously requires specific training and a certain degree of experience, it is relatively simple. In the course, the first thing we explain to our students is how the planning of this type of treatment is carried out by analyzing the computerized tomographies, the intra- and extraoral photographs and the analysis of the models previously taken. It must be taken into account, the aesthetic parameters, such as the smile line, facial profile, resting lip position, etc. The team of Anesthesiologists of the CIMIV Institute also allows us to perform these procedures under intravenous sedation, so it is completely painless, with minimal postoperative discomfort and the patient has the security of a hospital environment and a team with the maximum level of professional competence.
