Patient complains of gradual onset of pain in the right loin for last 1 year. The pain is dull aching in nature and persisted almost continuously for that period, sometimes patient has relief after passage of urine and with analgesics. Patient noticed a lump in the right loin for last 1 year, gradually increasing in size. There is no history of rapid increase in size of the swelling and there is no history of diminution of size with passage of urine. No history of hematuria. No history of difficulty in passing urine. Patient passes urine 5–6 times in 24 hours. No history of increased frequency of micturition. No history of fever. Patient has no other symptoms. On physical examination, general survey is essentially normal except slight pallor. Blood pressure is 130/80 mm Hg. On local examination of abdomen, the inspection of the contour of abdomen and umbilicus are normal. There is slight fullness in the right lumbar region; also the right renal angle is full. On palpation temperature of the abdomen is normal, there is no tenderness. The right kidney is palpable, reniform in shape, 9 cm × 6 cm in dimension, moving up and down with respiration. There is slight mobility of the swelling from side to side and from above downwards, the medial lateral, upper and lower margin of the swelling is palpable and rounded, surface is smooth and consistency is tense cystic, the hand can be insinuated between the swelling and the right costal margin. On percussion, the swelling is dull and there is a band of resonance in the front of the swelling. Liver/spleen are not palpable, no other lump is palpable, no free fluid in abdomen. Systemic examination is normal.
