I use cotton-tipped applicators a lot in surgery. I especially like to palpate structures to demonstrate obstruction. In this video, the applicator is used to demonstrate nasolacrimal duct obstruction and meibomian gland obstruction. Prior to performing a lacrimal surgery, I will confirm the area of obstruction prior to performing the surgery. For NLDO, I will palpate the area of the lacrimal sac. If there is reflux, then I am reassured that I am doing the correct surgery. For incision and drainage of chalazia, I also like to demonstrate if there is adjacent Meibomian gland obstruction. Again, if this is demonstrated, I am reassured that I am doing the correct intervention. This is Richard Allen at oculosurg.com. This video demonstrates the use of a cotton-tipped applicator to show areas of obstruction. The first is a pediatric patient with nasolacrimal duct obstruction. Palpating the lacrimal sac demonstrates reflux through the upper punctum, which confirms nasolacrimal duct obstruction. In the second video, the lacrimal sac is again palpated with reflux through the upper punctum. There is a little blood in the reflux which may make you suspect a tumor in the lacrimal sac. Lastly, meibomian gland obstruction is demonstrated in a patient undergoing an incision and drainage of a chalazion. Pressing on the glands demonstrates the thick contents of the meibomian glands.
