Application (CG – 1258) (this number is the USCG form number) You can download the application here : https://www.dco.uscg.mil/Portals/9/DC... 2. Simplified Measurements Form (CG – 5397) You can download the form here : https://www.dco.uscg.mil/Portals/9/DC... 3. A scanned copy of your boat's Title and State Registration 4. Credit Card Authorization Form (CG – 7042) You can download the form here : https://www.dco.uscg.mil/Portals/9/DC... And thats all you need !
