I am a board-certified cosmetic surgeon and I practice at The Gallery of Cosmetic Surgery in Lynnwood, Washington just north of Seattle. Today, we're going to discuss breast augmentation and breast surgery. Do we place the implants above the muscle do they go below the muscle and even a third is underneath the fascia. There's three potential places to put the implant. The majority of the time above the muscle is limited because all of the weight then is going to be added to the ligaments of the skin and the skin ligaments are going to not be as strong, not hold up the breast over time. It will usually facilitate leading to more of a droopiness or saggy-ness of the breast over a smaller period of time. So the move is to be putting them under the muscle to give more support to minimize that dropping of the tissues. There's now people placing implants underneath the fascia which is in between so instead of just underneath the muscle it's on top of the muscle but a thin layer of the tissue over the top of it so it's in between. The advantage is that it gives a little bit of extra coverage of the implant so you don't see it but then individuals who are very active with the flexing of their muscles of their pectoralis major muscles, they don't deform the implant. One of the disadvantages of being under the muscle is when they flex or move their chest, it could potentially change or alter the look of the implant and patients don't like that. It doesn't do it permanently, just when the activity or motion of the chest is happening. So, the placement under the fascia tends to reduce that with still giving enough coverage over the implant to reduce that affect.
