Uterine Fibroid Embolization for Symptomatic fibroids

Patient Profile: A young female initially presented with minor menstrual discomfort in January 2023. The condition escalated in February 2023, manifesting as the discharge of significant blood clots during the menstrual cycle. Initial Investigations and Treatment: Ultrasound was performed revealing an endometrial thickness of 18 mm, with endometrial polyp and small uterine fibroids. The patient sought medical advice, and non-hormonal medical management was initiated. The treatment initially offered relief; however, soon the patient's symptoms aggravated causing severe lower abdominal and back pain during menstruation, accompanied by heavy bleeding lasting 10-12 days. Repeat USG revealed increased endometrial thickness measuring 36 mm with increased size of uterine fibroid, and polyp. Surgical Intervention: In October 2023, surgical removal of an endometrial polyp was recommended due to the persistent symptoms and increasing size. Polypectomy with endometrial curettage was successfully performed. The patient was apparently alright for 2-3 months post-procedure. The patient again experienced significant pain and heavy bleeding in the subsequent menstrual cycles, leading to considerable blood loss. Further Complications: By January 2024, the patient's condition worsened, with unbearable pain and continuous heavy bleeding. Further diagnostics, including USG and dynamic MRI pelvis identified sub endometrial uterine fibroids largest measuring 66x48x62 mm with hemorrhagic transformation and non-responsive to medical management. At this point, uterine artery embolization was suggested as the next course of action.
