Echogenic Liver Mass on Ultrasound & Doppler: Is It a Hepatic Hemangioma? Key Findings & Diagnosis

A 4 cm inhomogeneous echogenic mass is seen in the right lobe of the liver, sub-diaphragmatically, with no vascularity on color Doppler. The patient is asymptomatic. The most likely diagnosis is hepatic hemangioma, a common benign lesion with slow blood flow, explaining the absence of Doppler signals. Differential diagnoses include focal fatty changes, hepatic lipoma, metastasis, and, less likely, eosinophilic abscess or hepatocellular carcinoma (HCC). HCC is improbable due to the lack of vascularity and absence of risk factors. Larger hemangiomas may appear inhomogeneous due to thrombosis or fibrosis. Further assessment with contrast-enhanced ultrasound (CEUS), MRI, or CT can confirm the diagnosis, showing progressive centripetal enhancement in hemangiomas.
