Here I show a mostly unedited vertical chopping technique for nucleofractis. Chopping is a useful nucleofractis technique. Chopping techniques are more difficult to learn than divide and conquer as the second hand takes on a bigger role. Additionally chopping requires increased awareness of machine settings and fluidics of the anterior chamber. Chopping reduces the amount of sculpting which typically results in less ultrasound time attenuating endothelial injury. Chopping is especially safe in soft lenses and in white cataracts. Chopping may be easier on the zonules. A slow transition is the best way to learn chopping. First learn divide and conquer where one handed techniques are possible and where your attending can do more for the second hand. Then transition with techniques like stop n chop where you start off with divide and conquer and then after dividing in half chop into quarters. We use Kitaro and Phillips PS-12 artificial eyes to simulate chopping. Here is an example of Drs. A Tim Johnson and Austin Fox at University of Iowa using Phillips PS-12 eyes for nucleofractis practice -- • stop n chop with phillips eye Chopping to some extent requires increased awareness of machine settings. In order to get a good hold on the nucleus you want the phaco needle embedded into a tight hole in the nucleus. I suggest using pure longitudinal to bore into the nucleus. If you use torsional (side to side) ultrasound the hole into the nucleus will be bigger than the needle tip. If the needle is tightly embeded the vacuum at the needle tip will better hold the nucleus during chopping. When the nucleus is harder as in this case the motion with the chopper is more vertical. When the nucleus is soft the motion is more horizontal such as in this case -- • Видео .
