The anterior approach to the ankle joint is a surgical technique used to gain access to the joint from the front of the ankle. It provides excellent exposure of the ankle joint for procedures such as arthrodesis (joint fusion) and the treatment of anterior lip fractures of the tibia. The approach involves making a longitudinal incision over the anterior aspect of the ankle joint, approximately 15 centimeters in length. The incision begins about 10 centimeters proximal to the joint and extends so that it crosses the joint midway between the medial and lateral malleoli, ending on the dorsum of the foot. Care must be taken to cut only the skin during the incision, as the anterior neurovascular bundle and branches of the superficial peroneal nerve cross the ankle joint very close to the line of the skin incision. Once the incision is made, the surgeon proceeds with deepening the dissection by incising the deep fascia of the leg in line with the skin incision and then incising the extensor retinaculum. The structures at risk during this approach include the superficial peroneal nerve cutaneous branches and the neurovascular bundle consisting of the deep peroneal nerve and anterior tibial artery. You can make a step cut if needed to facilitate repair. Repairing the retinaculum is critically important as it avoid bowstring. The anterior approach provides access to the medial malleolus and the medial articular facet of the body of the talus. It is commonly utilized for ankle fusion procedures and addressing specific fractures or pathologies of the ankle joint. During the procedure, osteophytes may be removed, and joint surfaces can be prepared. Additionally, an ankle arthrodesis plate can be positioned and used for alignment improvement. In summary, the anterior approach to the ankle joint involves a longitudinal incision over the anterior aspect of the ankle, allowing access to the joint from the front. It is a valuable technique for ankle fusion and addressing certain ankle fractures or pathologies.
