Old trauma + Cornea scar + Corectopia + Membranous cataract + Pupil reconstruction + RPE atrophy

This patient suffered an ocular trauma almost 30 years ago in his childhood. Since then he hasn't been able to see with this eye. A vascularized corneal scar might be seen, indicating whhich region was affected. Considered the old sinechia and bad visualization of the area, the decision was to use the vitrectome probe to open a new pupil, remove the central part of the membranous cataract (which demanded 23G scissors) and suture the periphery of the iris. A small hemorrhage was impossible to avoid. After opening a the visual axis the posterior pole could be seen. RPE seemed to be atrophic (check its gray aspect) and the clot was left in order to avoid a vitrectomy at this moment.
