This woman was suffering from chronic migraine headaches and hemiplegic migraines that would cause her to be paralyzed on the left side of her body temporarily. This would occur when exposed to loud sounds and she also experienced a third window syndrome (superior semicircular canal dehiscence syndrome). She had sound-induced dizziness and nausea as well as autophony. This video describes third window syndrome (also referred to as otic capsule dehiscence syndrome), the associated symptoms and what this patient experienced before and after her surgery. She had recurrence of her symptoms first in her right ear and then her left, both resolved with round window reinforcement surgery. When she recurred again a detailed search for co-morbidities affecting her surgical outcome revealed that she also had functional neurological symptom disorder (conversion disorder) as well as traumatic brain injury.
