Pes Anserine Located two to three inches below the knee cap on the medial aspect of anterior tibia. It is the spot where tendons of three large thigh muscles attach to the tibia. Sartorius muscle is an anterior thigh muscle that attaches to the upper, medial aspect of the tibia. The gracilis is a medial thigh muscle that connects to the tibia just below the sartorius tendon. The semitendinosus is a hamstring muscle. Its tendon attaches to the tibia just below the attachment of the gracilis. The conjoined attachment of the three tendons resembles the webbed foot of a goose. Therefore, pes anserine bursitis is nicknamed “Goosefoot Bursitis.” A bursa is a fluid filled sac that reduces friction. They are filled with synovial fluid. Bursa sacs are located between the surfaces of two structures that rub together. Most often these structures are either a bone and a muscle or a bone and a tendon. Bursa sacs function to improve movement by reducing friction. The pes anserine bursa is located between the sartorius, gracilis and semitendinosus tendons and the tibia.
