Performing cataract surgery in a RK patient Pearls: Incision, IOL calculation etc Kanellopoulos, MD

In this case we describe along an uneventful clear-cornea phacoemulsification cataract extraction in a post-Radial Keratocomy (RK) patient our in-house methodology for IOL calculation, especially in regard to the amount and axis of cylinder used for a toric IOL (a T6 Acrysof specifically). We of course always run with the the ASCRS formula that in this specific case provided a 2 diopter range (!!! which one to choose?) . We actually refined the IOL power quest with our empiric formula of calculating the amount and axis of cylinder from a virtual topography-guided plan with Alcon's Contura software: the amount and axis of suggested cylinder correction for a 5.5mm OZ-shown in the video (in the US you can only go down to 6mm) is then used in the ASCRS post-RK IOL calculation formula (shown here). Additionally we calculate in our own empiric technique the IOL with the cylinder numbers noted above and the Lenstar measurements as if it was a "virgin" cornea, then add 2 Diopters of IOL power: this has been our go-to empiric formula, time proven but always confirmed on the ASCRS on-line calculator-Hope you find this video interesting-Regards A.
