Dr. Ebraheim’s educational animated video describes the anatomy of the extensor indicis muscle. The extensor indicis is part of the extensor dorsal compartment of the forearm. Origin:its origin is deep to the extensor digitorum communis (EDC) muscle, it originates from the posterior surface of the ulna and the interosseous membrane. Insertion: it inserts into the extensor expansion of the index finger. The extensor indicis muscle is deep and ulnar to the extensor digitorum communis muscle. The extensor indicis muscle belly extends most distally. It has the most distal belly of the fourth dorsal extensor compartment which is very helpful in the identification of this muscle when lacerations occur involving multiple tendons. This muscle is present in the fourth dorsal compartment of the wrist, in the video the image is a cross section showing all the 6 compartments and the different contents of each one. The extensor indicis tendon is ulnar to the extensor digitorum communis tendon. Innervation: the innervation of the extensor indicis muscle is supplied by the posterior interosseous nerve (which is part of the radial nerve). Function: it helps extend the index finger, and helps to extend the wrist. How do you examine the extensor indicis muscle? It is examined by having the patient make a fist and then extend the index finger. So the index finger has two tendons that extend it: Extensor indicis (EIP): locaed ulnar to the slip comes from the EDC. Extensor digitorum communis (EDC). The extensor indicis is sometimes transferred surgically to replace a torn extensor pollicis longus tendon. The extensor pollicis tendon sometimes becomes ruptured due to nondisplaced fracture of the distal radius, the patient cannot extend the thumb due to a ruptured EPL tendon. Repair will not be helpful. Tendon transfer is necessary. Tendon transfer from the extensor indicis (EIP). Preforming Tendon Transfer: When preforming tendon transfer, section the extensor indicis proximal to the sagittal hood to maintain independent extension of the index finger. In some cases of rheumatoid arthritis (RA), the extensor digitorum communis tendon to the index finger may have ruptured, and in this case, extensor indicis transfer is contra-indicated. Another case of tendon transfer of the extensor indicis is: Median nerve injury: extensor indicis opponent plasty, another spare tendon that can go to the thumb.
