Traditionally macular hole closure can be achieved with ILM peeling, gas tamponade and head positioning. ILM flaps and other techniques, such as amniotic membrane and autologous retinal transplantation, can be employed for large or recalcitrant macular holes. PFCL as an adjuvant has been described for macular hole repair associated with retinal detachment, particularly in highly-myopic eyes. Recently a novel surgical technique for macular hole closure was reported by Sabatino and co-authors in the May 2025 edition of Retina whereby PFCL was used to cover the macular hole and passive aspiration of fluid over the macular hole resulted in intra-operative and post-operative closure. This is a technique first described by Dr. Marzio Chizzolini at the 2017 FLORetina meeting. In today’s episode, returning guest surgeon Dr. David Perez Gonzalez shows us this technique, which he routinely uses for macular hole closure. Thank you Dr. Perez Gonzalez for sharing this case!
