Ayak parmaklarında iyileşmeyen yaraları olan diyabetik hastanın yapılan periferik anjiyografisinde anterior ve posterior tibial arterin total tıkalı olduğu görüldü. Posterior tibial arter distalde subintimal ilerlendi ve sonunda perfore oldu. Bunun üzerine anterior tibial arter cto segmenti antegrade olarak geçilip pedal loop yoluyla posterior tibial arter distal cto segmentine ulaşılarak subintimal alanda balon anjiyoplasti yapıldı ve antegrade yoldan posterior tibial arter lümenine düşülebildi. Yani CART tekniği kullanılarak hem anterior hem de posterior tibial arterde tam açıklık sağlanarak işlem pedal loop anjiyoplasti ile başarılı olarak sonlandırıldı. Thereupon, the anterior tibial artery cto segment was passed as antegrade and the posterior tibial artery distal cto segment was reached through the pedal loop, balloon angioplasty was performed in the subintimal area and the posterior tibial artery lumen could be reduced from the antegrade route.
