Since the 2019 update to the guidelines for the early management of acute ischemic stroke, there has been a large push to expand the use of endovascular thrombectomy (EVT) to more populations. Hazem Shoirah, MD, Icahn School of Medicine at Mount Sinai, New York, NY, highlights the use of EVT for large core infarctions as a notable area of development. Numerous studies have demonstrated favorable outcomes with EVT compared to medical management in large infarct core volume strokes, although higher rates of symptomatic intracranial hemorrhage (sICH) and any ICH have been reported. Since these large core strokes are associated with more damage, this development heightens the need for novel neuroprotective agents and strategies to mitigate ischemia-reperfusion injury. This interview took place at the American Academy of Neurology (AAN) Annual Meeting 2024 in Denver, CO.
