Platysmal bands are the vertical lines or ridges that appear on the front of the neck, often becoming more prominent with age. While many people assume these bands are just loose or sagging skin, they’re actually caused by the inner edges of the platysma muscle becoming visible as they pull apart over time. So, what exactly is the platysma muscle? This thin sheet of muscle starts just above the jawline and drapes down the front of the neck, attaching near the collarbones. When we’re younger, the platysma muscle is usually firm, elevated, and connected in the center of the neck, creating a smooth, tight contour, like a supportive hammock. However, as we age, gravity, genetics, and muscle laxity contribute to the platysma separating in the middle. When the inner edges weaken and drift apart, they begin to show through the skin as platysmal bands, which many people find bothersome or aging in appearance. This is where a platysmaplasty comes in. Platysmaplasty is a surgical procedure specifically designed to correct and repair these platysmal bands by tightening and repositioning the muscle. During a neck lift that includes platysmaplasty, a small incision is made discreetly under the chin. Through this opening, we carefully lift the skin to expose the platysma muscle and identify the separated bands. Now, not all platysmaplasty techniques are the same. Some surgeons simply stitch the bands together, which may seem like a quick fix, but this method can often lead to recurrence, the bands separating again over time. To achieve more lasting platysmaplasty results, our approach is more advanced and methodical. We begin by gently releasing the muscle underneath, which reduces unnecessary tension and makes it easier to bring the bands back to the center without excessive pulling. If needed, we conservatively trim any excess muscle tissue to improve the fit. Then, we secure the bands together using figure-of-eight sutures, a specialized stitching method that locks the muscle in place more reliably than standard techniques. Even with the best surgical technique, it’s important to acknowledge that some patients may still experience recurrence of platysmal bands. The platysma is a dynamic muscle involved in facial expressions and daily movement, so there’s always a small risk that the bands might eventually reappear. However, with refined methods and years of experience, we’ve significantly improved the durability of our platysmaplasty results and minimized the need for revisions. Ultimately, a successful platysmaplasty is about more than just sewing the muscle together. It requires a deeper understanding of anatomy, gentle muscle release, thoughtful trimming, and strong, precise suturing to truly restore the youthful neck “hammock” appearance.
