Tip-141; Cellular fibroma of the ovary is a benign stromal tumor composed of fibroblastic cells within a variably collagenous stroma. Patients usually present with symptoms related to a pelvic mass, as in the current case, and the tumors are unilateral. The tumors are solid but can have a cystic component. The tumor is composed of intersecting cellular bundles of bland spindle cells. It is characterized by bland nuclei, 3 or fewer mitotic figures per 10 high-power fields, and a low malignant potential. In contrast, cellular fibromas with 4 or more mitotic figures per 10 high-power fields are called mitotically active cellular fibromas and usually have a benign clinical course. In around 11% of cases, ovarian adhesions and extra-ovarian involvement secondary to the adhesions can occur, which might be associated with late recurrences. This is not considered metastasis, and appropriate clinical correlation is warranted. Remember that to call an ovarian tumor fibrosarcoma, there should be severe nuclear atypia, in addition to 4 or more mitotic figures per 10 high-power fields, otherwise, you cannot render this diagnosis.
