Which one happened to you? • • Four main reasons why patients present with pectoralis major tendon tears that were not repaired: 1. **Underestimating the Injury**: Initially, the injury does not seem severe. The patient experiences a characteristic painful pop or rip in the chest, accompanied by bruising of the arm or chest wall. After a few days, the pain and swelling subside. The shoulder begins to move normally, leading the patient to mistakenly believe they were not seriously injured. Over time, deformity and weakness become apparent. 2. **Myotendinous Tear that “cannot be repaired” **: This is the most common reason. The patient presents with an obvious injury, and an MRI reveals a pectoralis major tear, specifically a muscular or musculo-tendinous tear, aka a Myotendinous tear . The patient is incorrectly informed that it is a muscular tear that cannot be repaired. 3. **”Negative” MRI**: The patient presents with an obvious injury and bruising around the upper arm. An MRI of the shoulder is ordered under the suspicion of a biceps tear or shoulder injury. However, the MRI does not detect the pectoralis major tendon tear because the wrong MRI was ordered. Should have been Chest Wall MRI with pec major tendon protocol. 4. **”Partial Tear” Misinterpretation**: The patient presents with an obvious pectoralis major tendon tear. The MRI report indicates a partial tear, which is actually an under estimation. In reality, it is a full-thickness partial-width tear of the lower pectoralis major, leading to significant concavity deformity over time. However, the patient is incorrectly reassured that a partial tear is not a serious issue. • No matter what reason, there is hope because no matter how old it CAN be reconstructed. The surgery requires an understanding of the complex anatomy and additional resources, including specialized cadaver grafting techniques.
