Patient came to our clinic with the complain of pain and swelling in upper left central incisors since three months. Patient met with an accident three years before and he has got endodontic treatment for his tooth and a metal ceramic prosthesis was fabricated for it. On oral examination, tender on percussion was present and the radiographs showed periapical lesion with external root resorption. Considering the age of the patient immediate implant was planned. In the surgical phase we extracted the tooth and a full thickness flap was raised and the infected site and the periapical infected tissues were cleaned thoroughly. Straumann Implants - SLA SLActive Roxolid was then prepared for the immediate implant placement , an implant of size 4.1*12mm was placed in the extracted site. Proper care was taken to prepare the site maintaining the palatal position of the implant to retain a gap of at least 2mm between the implant and the labial plate. The labial wall defect was grafted using cerabone granulate bone graft and a bioguide collagen membrane (30-40mm) was placed over the grafted tissue.
