Sometimes a perfectly normal foreceps rhexis can turn rogue. When the anterior chamber is taut with Viscoelastic, the anterior lens capsule is flattened, facilitating a perfect forceps rhexis. But as we go ahead in this video we can see the rhexis running out of field. In such a case we re-flatten the convexity in the area of the run out and resume the case with a microforeceps. That does two things- it prevents excessive chamber collapse and it allows us to pull the rhexis flap centrally rather than circumferentially, to complete it and avoid a certain mishap We end up with a smaller rhexis here which we enlarge at the end of IOL implantation. The challenge is to place the IOL in the bag through a 3.5 MM rhexis. As we watch, the closed haptic single piece IOL enter the bag, we keep a second instrument ready to guide it in and successfully bag the IOL. Watch the video for hidden tips and tricks!!
