Surgery has been the traditional treatment of choice to diagnose and repair tubal abnormalities. However, this really has limited success for all but minimal and mild tubal abnormalities. As the alternative treatment for tubal disease, IVF, has markedly improved its success rates, surgery is done less and less. Nevertheless, if surgery is done for the diagnosis, then it is recommended to cut any scar tissue, burn any endometriosis and generally try to improve the pelvis. If one tube is closed and dilated (called a hydrosalpinx), removal of that tube may improve pregnancy rates, both naturally and with IVF. If both tubes are blocked, it may be necessary to remove both tubes. Repair of a damaged tube allows for the possibility of a spontaneous conception, but it is associated with an increased risk of a pregnancy in that tube (ectopic).            IVF has replaced surgery as the primary treatment of choice for tubal disease. IVF pregnancy rates today are much higher and faster than after tubal surgical repair, especially for diseased tubes. On the other hand, if tubes were “tied” for sterilization, one may prefer tubal reversal because it has high success rates and allows for spontaneous conception without the use of hormones. Usually it is associated with singleton. This is still a viable option for young women who may want more than one child. For those between 35 and 40, with reduced fecundity anyway, IVF may be statistically easier and faster. For women over 40 who are not good responders, IVF may have such a low chance of success; reversal may at least afford these women the option for spontaneous conception each month, even if the chance is statistically low. Based on your individual circumstances, it is important to evaluate all of your options to best decide the right option for you. The process of In Vitro Fertilization (IVF) requires a lot of attention to ensure the highest possible success rate. Initially, the woman is given fertility drugs that help the ovary grow eggs.  These are usually given as injections because the hormones made by the pituitary gland are proteins, and technology thus far has not been able to put these hormones in pill form without the stomach acid destroying them before absorption. During this time, the woman visits the clinic frequently for ultrasounds documenting the response of the ovary and counting the size and number of follicles that are growing. This is usually accompanied by blood tests in order to measure her hormone levels. The woman takes these fertility injections for an average of 8-12 days. In addition to fertility hormones that make the eggs grow, she also needs to take a medication to prevent the eggs from ovulating prematurely.
