There can be a bag of surprises hidden behind a total congenital cataract, few of which can be predicted using a preoperative ultrasound b scan. One such instance is finding a persistent hyperplastic primary vitreous (phpv) stalk attached to the posterior part of the total cataractous lens. This is a case of a 2year old child who presented with a unilateral mature cataract.
