A variety of methods have been developed in an attempt to repair articular cartilage defects. None are perfect. These include bone marrow stimulation techniques (subchondral drilling, abrasion, microfracture), osteochondral grafting (mosaicplasty) and autologous chondrocyte implantation (ACI). In addition, autologous chondrocytes have been cultured on collagen membranes prior to re- implantation, a technique known as MACI (matrix assisted autologous chondrocyte implantation). These procedures are limited to small and medium sized focal chondral and osteochondral defects. There is no evidence that the above techniques can reliably produce long lasting, durable articular cartilage.
