Definition:- Achalasia is "Esophageal motility disorder'' with 3 major abnormalities. 1- Increased resting tone of lower esophageal sphincter. 2- Aperistalsis (loss of peristalsis) in body of esophagus of distal 2/3rd. 3-Failure of lower esophageal sphincter to relax with swallowing. Pathogenesis 1-It results from degeneration of inhibitory neurons in myenteric plexus. 2-Inhibitory neurons contain nitric oxides & vasoactive intestinal peptide. Causes of Achalasia. 1- Idiopathic 2- Amyloidosis 3- Sarcoidosis 4- Diabetic autonomic neuropathy 5- Chagas disease 6- Pseudoachalasia = Achalasia like symptoms due to cancer of gastroesophageal junction. Presentation of Achalasia a) Dysphagia / difficulty swallowing. b) Dysphagia is for both Solids & Liquids. c) Weight loss. d) Nocturnal cough & regurgitation. e) Heartburn doesnot occur because of closed esophageal sphincter prevent reflux. Diagnosis of Achalasia Barium Swallow 1- Barium Swallow shows bird-beak appearance. 2- Esophageal dilatation with uniform tapering of distal esophagus. Esophageal Monometry 1- It is most accurate test. 2- It shows decrease peristalsis in body of esophagus. 3- It also shows increased resting pressure in lower esophageal sphincter. Chest X-ray 1-It shows abnormal widening of esophagus. 2- Chest x-ray is neither sensitive nor specific test. Upper Endoscopy 1- It shows normal mucosa in Achalasia. 2- It is usful to exclude malignancy.
