The following video describes cast mobilization post lapoplasty, as well as what is required postop, expected progression from a slippery cast into a below the knee cast. Also discussed ring, emotion, therapy, physical therapy, and progressing to weight-bearing. The video is intended to provide our patients direction as to what is expected. We frequently try to accelerate recovery when bone healing is visible. If bone healing seen early and radiographically, will start weight-bearing earlier than four weeks postop. First three weeks patient is essentially nonweightbearing with a slipper cast physical therapy starts at three weeks postop shifting to the removal cast for the next three weeks. The patient is using crutches for ambulation or a roll about. Always recommend her patients use crutches and helps with upper body strength and maintain physical fitness. The rollabout is beneficial for longer distances when crutches are not practical. We switched to three weeks of mobilization with Repair cast walker. At the end of six weeks, we tend to have a patient return to a running shoe with crutches, until the obtain a heel to toe progression. Once that is achieved, patient is allowed to continue walking in the running shoe all the time. Patient continues with strengthening and physical therapy program to attain full range of motion a great toe joint.
