This is the final video in a series designed to help provide a solid foundation for cataract surgery technique. With a consistent approach to the correct set up, you can make your surgery a lot easier. Positioning the head, with the cheek and brow level, puts the eye in the middle of the palpebral fissure with the patient looking directly up to the microscope light under topical anaesthetic. Keep the light levels low - try lying down on the operating table to experience for yourself the light levels that you are using. Lower light intensity makes surgery more comfortable for your patients, helping them to look directly up to the microscope light. This, and learning to float in your entry sites rather than lifting the eye into the nose, helps to keep the eye in the position of maximum visibility, and maximum visibility = maximum safety. Sustain a confident and reassuring 'verbal massage' throughout. This is particularly important when draping. It is at this stage of surgery, as in the example we show here, that patients tend to be most anxious and prone to squeezing the eyes shut. It's worth reminding the patient before you start of the two things that really help the surgery to go smoothly: "ordinary blinking is no problem, but try to keep both eyes open and avoid forceful squeezing"; "try to keep your head reasonably still once we have positioned it for you."
