Four Cases of Adenomyosis || Adnexal Cyst ||Globular Uterus Adenomyosis is a cause of heavy painful menses. The sonographic diagnosis is difficult to make. Poorly defined borders, minimal mass effect, an elliptical instead of globular shape, US shows areas of decreased echogenicity with heterogeneous myometrium. The junction between endometrium and myometrium is poorly defined. Case 1 Adenomyosis with Right adnexal cyst Myometrium: The myometrium diffusely heterogeneous- appearing without any discrete mass or contour deformity. The endometrial-myometrial border poorly defined. Right Adnexa : There is a fairly large cystic mass is noted in the Rt adenexa. Septal nodularity is marked however color Doppler shows no vascularity. Measuring 3.4cm. Left Ovary: Normal in size, and echogenicity. Measuring Volume 2.4 cc. Cul-de-Sac: Free from any fluid collection. Case 2 Adenomyosis Uterus: Bulky in size, globular in shape and anteverted in position. Myometrium: The myometrium diffusely heterogeneous- appearing with a discrete mass posterior myometrium measuring 5.4 x 4.7cm and contour deformity. The endometrial-myometrial border poorly defined. Case 3 Adenomyosis Uterus: Is mild bulky in size , and anteverted in position. Measuring 9.4 cm x 4.1 cm x 5.9 cm. Endometrium: Normal in thickness measuring 8.8mm. Myometrium: The myometrium diffusely heterogeneous- appearing without any discrete mass or contour deformity. The endometrial-myometrial border poorly defined with mild collection within the endometrial cavity ands also noted at the lower uterine cavity as well as cervical canal.
