However, as evidenced by thousands of testimonials, recent literature on quality of life outcomes after FGCS highlights the considerable impact it may have in the overall well-being, of female identifying patients assigned male at birth. The current growth in demand may also be due in part to the fact that this discipline addresses a crucial highly visible aspect of gender identity: the face. The World Professional Association´s best practice guide, the WPATH Standards of Care, in its 7th edition, raises the dilemma that facial feminization may be perceived as medically necessary yet more explicit recommendations are still needed. Although it is a personal decision dependent on factors such as the gender identity of the individual, level of social exclusion, exposure to discrimination and other circumstances, research increasingly supports the inclusion of facial gender affirming surgery as a valid option for those who elect surgical transition. The authors of this letter recommend that the WPATH SOC8 committee consider new investigation on the role of facial gender confirming surgery as an integral part of comprehensive gender-affirming care. Inclusion in the SOC8 would stimulate further studies and a more global approach to the health care of gender diverse patients, still a rapidly developing field with frontiers remaining to be fully explored.
